• Enrollees who are 18 years of age or younger are required to provide proof of full immunization against the hepatitis B virus prior to enrollment

Current immunization recommendations* advise students 26 years of age or younger to be fully vaccinated against human papillomavirus (HPV) before first-time enrollment at a California State University, University of California or California Community College campus.

HPV vaccination can prevent over 90 percent of cancers caused by HPV. HPV疫苗是非常安全的, and scientific research shows that the benefits of HPV vaccination far outweigh the potential risks.

• Outbreak risk reduction: 流感; measles, mumps and rubella (MMR); meningococcal conjugate (serogroups A, C, Y, W-135), serogroup B meningococcal; Tetanus-diphtheria-pertussis (Tdap), and varicella (chickenpox) vaccines.

Additional vaccines may be required: 

• For students enrolled in health care fields of study or teacher education programs will be required to provide immunization completion documentation as required by state licensing agencies or clinical placement or fieldwork sites.

• In the event of a local or campus outbreak of a vaccine-preventable disease, the Los Angeles County Department of Public Health and/or the California Department of Public Health are authorized to require immunizations beyond those required by the CSU.

All required documentation must be submitted through the 病人门户.

All students are required to submit acceptable proof of full immunization. Acceptable proof is defined as a written document that includes immunization names and dates (e.g., immunization record) or dates and results of blood testing indicating immunity (e.g., lab report) that is signed and dated by a licensed healthcare provider.

Please note: The 学生健康中心 is only able to accept documents in English. Records in other languages must be translated into English by a licensed healthcare provider prior to submission to the 病人门户.

Incoming students submit the required immunization records through the 病人门户.

• Click on the "Clearances and 形式" tab.

  • Enter immunization and dates each immunization dose was received.

  • Upload immunization record documents. Be sure documents meet the definition of acceptable proof.

  • Additional records may be uploaded at any time, regardless of compliance status.

学生 can check the Clearances and 形式 tab for compliance status updates. 学生 will receive a message through the 病人门户 if any items are missing or require further action.

Obtaining Services Through the 学生健康中心

学生 who would like to fulfill the immunization requirement (or department-required lab or TB screening) through the 学生健康中心 should call 323-343-3300 to schedule an appointment.

学生 new to the University may complete 加州大学洛杉矶分校 required immunizations, 结核病筛查, or related lab tests prior to their first semester of enrollment under the following conditions:

• Services are available only during the term immediately preceding the first semester of enrollment.

• Proof of acceptance into the University and program enrollment verification must be shown prior to receiving services.

• 学生 must pay applicable immunization and lab fees.
